STEP 3- Review Statement of Policy for the Protection of Children and Youth and the Code of Conduct for Church Personnel

In signing the “Application for Volunteer Services” form, you affirm that you have received and reviewed a copy of both the Statement of Policy for the Protection of Children and Youth and the Code of Conduct for Church Personnel.

STEP 4- STAND Safe Environment Training

Volunteers Age 18 and Up Must Complete the Online STAND Training.  Upon conclusion of the course, you will need to print and submit a certificate of completion.

STEP 5- Completed Forms
Please send all completed forms (Application, Criminal History Consent, & Training Video Certificate) to the Parish Office, Attn. Marianne Faulstich, STAND Coordinator. You may drop them off, mail them (12500 Clarksville Pike, Clarksville, MD, 21029) or fax them in (410-531-6191).

OPTIONAL- Driving Record Check Consent Form
This form would be for parents who are volunteering to drive as part of service days, field trips, etc.
