In signing the “Application for Volunteer Services” form, you affirm that you have received and reviewed a copy of both the Statement of Policy for the Protection of Children and Youth and the Code of Conduct for Church Personnel.

The Archdiocese of Baltimore’s safe environment (STAND) training is available online. Upon successful completion of the course, you will be able to print a certificate of completion.

STEP 6- Completed Forms
Please send all completed forms (Application, References, Criminal History Consent, & Training Video Certificate) to the Parish Office, Attn. Marianne Faulstich, STAND Coordinator. You may drop them off, mail them (12500 Clarksville Pike, Clarksville, MD, 21029) or fax them in (410-531-6191).
OPTIONAL- DRIVING RECORD CHECK CONSENT FORM
This form would be for parents who are volunteering to drive as part of service days, field trips, etc.
