SUBMIT BAPTISMAL INFORMATION BY MARCH 1

If you did not upload a Baptismal certificate above, please submit a copy of the candidate’s Baptismal Certificate to the CYM Office no later than March 1. You may need to contact your child’s Church of Baptism, please allow time to do so and meet the March 1 due date. 

You may email a scanned copy of the form to CYM Office Manager, MC Tenaglia.
