2022-2023 Health Forms
Parents should use the School Admin Portal to complete all necessary health and immunization forms. The forms listed below can be uploaded into your School Admin portal or can be printed, completed, and mailed/dropped off to the school office by July 31, 2022.
Mandatory for all students:

Health Assessment for Grades Pre-K - 8 - Part I and II of the State of Maryland Health Assessment
Immunization Certificate - Maryland Department Health and Mental Hygiene Immunization Certificate
Mandatory for all 6th grade students:
Health Assessment for Grade 6 Part I and II of the State of Maryland Health Assessment
Mandatory for all 7th grade students:
Immunization Certificate - Maryland Department Health and Mental Hygiene Immunization Certificate
