    Registration Information

  • The fee for the Confirmation program is $115 due MARCH 1, 2021, unless other arrangements have been made with the Confirmation Coordinator. This fee covers retreat materials, robe rental, instructional materials, etc. If retreats will be IN PERSON, there will be an additional fee, explained at the Information Session.
    There are two options for payment:
    1) If you choose to pay by check, make it payable to St. Louis Parish and mail it to St. Louis CYM CONFIRMATION PROGRAM, 12500 Clarksville Pike, Clarksville, MD 21029.
    2) If you choose to pay by credit card, an online payment link will appear when you submit this registration form.
    No credit cards can be taken after Mar. 1.
    No one will be turned away for financial reasons. Contact the CYM office if finances are a concern for you.
    No refunds will be issued after March 1 if candidate voluntarily withdraws from the program.

2. SACRAMENTAL RECORDS FORM
SUBMIT BAPTISMAL INFORMATION BY MARCH 1, 2021

To complete your registration, you must also complete the Sacramental Information Form BELOW, and submit a copy of the candidate's Baptismal Certificate to the CYM Office no later than March 1, 2021. You may need to contact your child's Church of Baptism, please allow time to do so and meet the March 1 due date.
